How Do I....
Register With The Practice?
We are only able to provide you with urgent or emergency medical services until you have joined the practice. We try to keep the joining (or “registration”) process as simple as possible. But, there are some things to do so that we can get your medical notes from your last doctor.
You do not need an appointment to register. Just come to reception at your most convenient surgery.
- If you have your NHS medical card, please sign it and bring it with you. If not, we have a simple form for you to sign.
- We will help you fill in a questionnaire about yourself, your health and medical history. This helps us to give you the best care.
- We would then like you to book to have a brief check-up with our practice nurses. This includes blood pressure, weight and a urine test.
Once the registration form, questionnaire and check-up are done, we can register you with the Primary Care Trust as having joined the practice. We will then be able to offer you the whole range of NHS health services.
Both practices are accessible by wheelchairs to all clinical areas and both have disabled toilets.
